LPN - Primary Care Specialist

About the Role:
CenterWell Senior Primary Care is seeking a skilled LPN to join our team in Gastonia. As an LPN, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities:
- Assess patient health problems and needs, taking vital signs and administering medication as needed.
- Develop and implement nursing care plans to ensure patients receive comprehensive care.
- Work collaboratively with integrated care teams, including physicians, nurses, and behavioral health specialists, to provide compassionate, personalized care.
- Advise patients on health maintenance and disease prevention, and provide case management as needed.
Requirements:
- Current licensure as an LPN or LVN.
- Excellent customer service skills and a strong clinical background.
- Ability to work in a fast-paced, high-volume environment and prioritize tasks effectively.
- Passion for contributing to an organization focused on continuously improving consumer experiences.
Preferred Qualifications:
- Bilingual skills.
- Hands-on professional experience.
- Phlebotomy experience.
- Experience with Electronic Medical Records and HEDIS.
