Position Overview
We are seeking a dedicated Physical Therapist to join our dynamic team. This role involves working closely with a supportive rehabilitation team, committed to restoring function and preventing further decline in patients' abilities to engage in meaningful daily activities.
Key Responsibilities
- Assess resident information and determine its influence on the evaluation and treatment process.
- Conduct comprehensive functional assessments of residents using standardized methodologies.
- Design and execute personalized treatment plans based on individual resident needs, incorporating achievable functional objectives.
- Adhere to evaluation, treatment, and documentation protocols while maintaining accurate and timely records.
- Guide and supervise Assistants in treatment techniques and procedures.
- Document treatment sessions and progress notes in resident records after each session.
- Adjust treatment plans as necessary throughout the therapy process.
- Prepare clear and concise discharge summaries in the prescribed format.
- Introduce residents to program services and treatment methods suitable for their needs.
- Collaborate with nursing staff, residents, and families to optimize residents' functional capabilities.
- Educate residents and their families or caregivers on home exercise programs and therapeutic techniques to continue post-treatment.
- Assist in referrals for additional services required after discharge and maintain knowledge of community resources.
- Participate in rehabilitation team meetings to discuss resident progress, challenges, or needs.
- Engage in departmental meetings as required.
- Pursue continuing education opportunities relevant to your discipline to uphold clinical competency.
- Contribute to quality improvement initiatives.
- Comply with regulatory and professional standards set by Ahava Therapy Services.
- Follow facility policies and engage in quality improvement and safety programs.
- Perform additional duties as necessary to ensure high-quality care for residents.
- Understand reimbursement processes and allocate therapy resources effectively based on resident goals and outcomes.
- Possess knowledge of relevant regulations and guidelines.
- Support the organization's mission and vision while maintaining a professional demeanor.
- Independently execute responsibilities and recognize when direct supervision is required, offering assistance to colleagues as needed.
- Meet the expected productivity levels for your role within the facility.
- Bachelor's or Master's degree in Physical Therapy.
- Current licensure in the state of practice.
- BLS CPR certification required upon hire or obtained during orientation, with ongoing maintenance during employment.
