We are seeking a compassionate and skilled Weekend Home Health Physical Therapist to join our team. As a Physical Therapist with Jefferson Health at Home by BAYADA, you will have the opportunity to make a meaningful difference in the lives of our patients, providing personalized care and support to help them achieve their goals and maintain their independence.
Responsibilities:
- Conduct thorough assessments of patients' physical abilities and develop personalized care plans to address their unique needs and goals.
- Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and interventions to promote optimal outcomes.
- Collaborate with other healthcare professionals, including nurses and social workers, to ensure comprehensive and coordinated care.
- Communicate effectively with patients and their families, providing education and support to promote understanding and empowerment.
- Document patient progress and outcomes, using standardized tools and measures to track and evaluate the effectiveness of care.
Requirements:
- Active and current license to practice as a professional Physical Therapist (PT) in the state of PA.
- Experience with OASIS strongly preferred.
- Ability to travel to cases as assigned.
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
