We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for ensuring the quality and resource management of all patients admitted to our facility.

Responsibilities:

  • Conduct thorough assessments of patients to identify their medical, emotional, and social needs.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare providers to ensure seamless delivery of care and achieve optimal patient outcomes.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Provide education and support to patients and their families to promote healthy behaviors and self-care.

Qualifications:

  • Current state Registered Nurse license.
  • Minimum 5 years of work experience post-graduation from an accredited school of nursing.
  • Experience in case management, discharge planning, or nursing management is preferred.
  • Knowledge of Milliman Criteria and InterQual Criteria is a plus.
  • Basic to intermediate computer skills are required.
