As a Clinical Pharmacist, you will play a vital role in providing pharmaceutical care to patients of all ages and population groups. You will work closely with nursing and medical staff to ensure appropriate prescribing, route, method of administration, and length of therapy. Your expertise will be essential in monitoring for age-appropriate response to drug therapy and identifying any adverse events from drug therapy.
Key Responsibilities- Provide pharmaceutical care to patients, including review of allergy history and weight-based dosing
- Monitor for age-appropriate response to drug therapy and identify any adverse events from drug therapy
- Perform computer order entry of medication orders and maintain patient medication profiles
- Conduct therapy monitoring and other activities associated with the provision of pharmaceutical care
- Provide guidance to technical staff on pharmacy services and supervise technical support personnel
- Perform accurate final dispensing check of all medication orders and complete required documentation
- Comply with applicable federal and state law, pharmacy standards of practice, and US Pharmacopeia guidelines for compounding of sterile preparations
- Participate in activities targeted by the department, including Clinical Pharmacy Reports, Medication Utilization Evaluation (MUE), food/drug interaction monitoring, patient counseling, etc.
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
- Minimum 1 year of pharmacy hospital experience preferred
- Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or licensed pharmacist in another state
