We are seeking a highly skilled Advanced Practice Provider to join our team at LiveWell Choice, LLC. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our members in long-term care settings.

About the Role:

The Advanced Practice Provider will work closely with our interdisciplinary team to develop and implement individualized care plans for our members. This will involve conducting comprehensive assessments, developing treatment plans, and providing education to members and their families.

Key Responsibilities:
  • Provide primary care services to a panel of Longevity Health members in long-term care settings
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Conduct comprehensive assessments and develop treatment plans to address the unique needs of each member
  • Provide education to members and their families on health and wellness topics
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
Requirements:
  • State unrestricted licensed Advanced Practice Nurse or Physician's Assistant
  • National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant
  • DEA License required
  • Active national provider number (NPI) and active Medicare provider number
  • Valid state driver's license with a good driving record and proof of automobile insurance
