Job SummaryWe are seeking a skilled Physical Therapist to join our team in a community setting. As a Physical Therapist with Pinnacle Home Care Inc., you will travel to patients' homes to administer services and provide a variety of tasks during patient visits depending on the specific plan of care and type of patient.
Key Responsibilities- Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Teaches, supervises, and counsels the family and client in the physical therapy program.
- Communicates with other team members and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
- Writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Home healthcare experience is preferred but not essential.
- Electronic medical record experience is preferred but not essential.
