Job Title: Regional Home Health RN Quality Assurance (OT, PT, or RN)

LHC Group is seeking a highly skilled and experienced Regional Home Health RN Quality Assurance professional to join our team. As a key member of our quality improvement team, you will be responsible for ensuring the highest level of quality care is delivered to our patients.

Key Responsibilities:
  • Facilitate overall agency quality improvement and outcome performance
  • Perform on-site survey readiness assessments and develop action plans to mitigate quality-related findings
  • Support agencies during state and Joint Commission surveys
  • Maintain current knowledge of regulations affecting the agency and provide guidance to leadership
  • Perform site visits to identify and address mitigating factors inhibiting improvement in quality performance
  • Assist agency leadership in prioritizing performance improvement activities and developing improvement plans
  • Monitor internal quality chart review results and provide feedback on effective action plan development
  • Perform inter-rater reliability audits on internal quality chart reviews
  • Provide orientation to quality and performance improvement program to new clinical and executive directors
  • Analyze data and quality reports to identify improvement opportunities and develop plans to improve overall quality performance
  • Assist agency leaders with root cause analysis and action plan development related to Sentinel Events and other identified patient safety risks
Requirements:
  • 3 years healthcare experience, preferably home care and/or healthcare quality-related experience
  • Bachelor's Degree in a healthcare-related field, preferred
  • Certification in Healthcare Quality is preferred, after first year of service
  • Understanding of CMS Conditions of Participation, Joint Commission Standards, and state licensing standards
  • Proficient computer skills with knowledge of Microsoft Word and Excel
