Key Responsibilities- Provide emotional support, mental health evaluations, and therapy to patients and their families.
- Advocate for patients and help them access resources to address personal issues.
- Counsel patients and provide crisis intervention, including assessment and treatment of behavioral problems.
- Coach patients on managing ongoing health conditions and provide therapy to promote positive behavior changes.
- Assist patients and their families through individual, group, and education conferences to understand and follow treatment recommendations.
The Social Worker will provide psychosocial support to patients, families, and caregivers, including advising family caregivers, providing patient education and counseling, and making necessary referrals for other social services.
The Social Worker will work with the multidisciplinary team to understand significant social and emotional factors underlying a patient's health and/or attitude problems.
Requirements- Master's Degree in Social Work
- Certification in relevant specialty or field
- Minimum 1 year of experience
- Bilingual (English/Spanish) highly preferred
