About the Role:

The successful candidate will be responsible for coordinating patient care, ensuring optimum outcomes, and promoting consistent positive patient interactions. As a RN Case Manager, you will act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status.

You will ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences. This role requires strong clinical skills, excellent communication abilities, and a commitment to providing exceptional patient care.

Responsibilities:

  • Provide program orientation to patients/families/caregivers, including the case manager's role, rehab philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions.
  • Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments.

Requirements:

To be considered for this role, you must have:

  • A current state RN license; BSN preferred.
  • Three years' clinical, hospital nursing experience required, with preference given to those with 2 years of case management experience or 2 years of critical care experience.
  • Certification in case management or utilization review preferred.
  • InterQual experience preferred.
