**Job Summary**

The Senior Healthcare Counselor will provide direct counseling and social work services to residents and their families, working collaboratively with our healthcare team to enhance patient health and wellbeing.

**Responsibilities**

- Provide individualized counseling and support to residents and their families

- Develop and implement personalized care plans to address the unique needs of each resident

- Collaborate with healthcare professionals to ensure seamless coordination of care

- Participate in regular team meetings to discuss resident progress and develop strategies for improvement

- Maintain accurate records and documentation of resident interactions and care plans

**Requirements**

- Bachelor's degree in Social Work (BSW) required

- Previous experience in healthcare or social work preferred
