Job Description

We are seeking a highly skilled Registered Nurse to join our team as a Clinical Care Coordinator in Wichita, KS. As a key member of our interdisciplinary care team, you will be responsible for providing direct nursing care to patients, implementing and monitoring patient care plans, and serving as a primary coordinator of all disciplines for well-coordinated patient care.

Responsibilities:
  • Provide direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.
  • Implement and monitor patient care plans, including assessing and coordinating patient discharge planning needs with members of the healthcare team.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care, ensuring seamless transitions between departments and services.
Required Skills and Qualifications

To be successful in this role, you must possess:

  • A current license as a Registered Nurse in the state of Kansas.
  • Experience in a hospital setting, preferably in a medical-surgical unit or critical care environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
