About the Role

CareOne is seeking a highly skilled Physical Therapy Assistant to join our team. As a key member of our interdisciplinary team, you will play a vital role in managing the care of our residents, ensuring quality outcomes, and providing efficient treatment plans.

Responsibilities
  • Provide and direct resident rehabilitation care, adhering to all relevant standards and regulations.
  • Evaluate resident needs and establish effective treatment plans, collaborating with other disciplines as necessary.
  • Manage and coordinate the supervision of treatment plans, ensuring seamless communication with staff and other departments.
  • Participate in resident screenings, providing feedback to supervisors on clinical and rehabilitation issues.
  • Contribute to center-directed processes and services, demonstrating effective management of treatment and non-treatment activities.
  • Maintain accurate and timely clinical documentation, meeting all relevant agency requirements.
  • Assist with marketing efforts, as directed.
  • Participate in in-service training programs, as requested.
Requirements
  • Bachelor's Degree in Physical Therapy
  • Current and Active Physical Therapy Assistant license within the state of operations
  • Prefer experience/knowledge in older adult rehabilitation
  • Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings
