About the RoleAs a Home Health RN Case Manager, you will provide high-quality, personalized care to patients in the comfort of their own homes. You will work closely with a team of healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement comprehensive care plans that address patients' goals and objectives
- Provide education and support to patients and their families on disease management, medication, and home safety
- Collaborate with healthcare teams to ensure seamless transitions of care
- Monitor patients' progress and make adjustments to care plans as needed
- A competitive salary range of $75,000 - $100,000 annually
- A comprehensive benefits package, including health, dental, and vision insurance
- A 401(k) retirement plan with company match
- Paid vacation time and mileage reimbursement
- Flexible assignments and autonomy to manage your caseload
- Active, unrestricted RN license in the State of Michigan
- At least one year of clinical experience as a Registered Nurse
