Key Responsibilities
  • Embrace the organization's mission by delivering excellence in all job aspects while fostering positive relationships with colleagues.
  • Evaluate, diagnose, and create tailored care plans for patients requiring palliative care.
  • Act as a key liaison for care coordination across the VNA continuum for patients with complex health issues.
  • Represent the agency in interactions with patients, families, and other healthcare organizations.
  • Comply with infection control and safety protocols set by the agency.
  • Follow organizational policies regarding attendance and professional appearance.
  • Provide care in various settings, including hospitals, facilities, and home environments.
Qualifications

Essential Skills and Experience:

  • Master's degree in Nursing from an accredited program.
  • Current licensure as an Advanced Practice Nurse (APN) in New Jersey with prescriptive authority; must maintain CDS and DEA certifications.
  • National certification as an Adult, Geriatric, or Hospice and Palliative Care Nurse Practitioner.
  • 1-2 years of experience as an APN is preferred.
  • Exceptional communication, organizational, and interpersonal skills.
  • Proficient in computer usage.
Working Conditions

This role involves working in both indoor and outdoor environments, with direct client contact and the use of clinical equipment, including phones and computers. A valid driver's license and automobile insurance are required.
