Care Coordinator RN

As a Care Coordinator RN at Kaiser Permanente, you will play a vital role in planning, developing, and evaluating care provided to patients. Your primary responsibility will be to provide case management and outreach services to selected patients, ensuring they receive the necessary resources and support to meet their needs. You will work closely with patients, families, and care providers to coordinate care, identify barriers, and develop strategies to improve outcomes. Your expertise in nursing and case management will enable you to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities:
- Develop and implement individualized care plans to address the unique needs of patients, including those with complex medical conditions and social determinants of health.
- Conduct thorough assessments of patients' needs, identifying gaps in care and developing strategies to address them.
- Collaborate with patients, families, and care providers to coordinate care, ensure continuity, and improve outcomes.
- Monitor patients' progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and care providers, using a non-judgmental and empathetic approach to build trust and rapport.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in case management and nursing, applying this knowledge to improve patient outcomes.
- 2+ years of direct patient care experience, preferably in a case management or clinical setting.
- Bachelor's degree in nursing or equivalent experience.
- Current RN licensure in Hawaii.
- Basic Life Support certification from the American Heart Association.
