Position Overview: The Hospice RN plays a vital role in evaluating and addressing the needs of patients and their families. Utilizing the nursing process, you will coordinate the Plan of Care with the Interdisciplinary Group (IDG) while delivering compassionate palliative and supportive care.
Key Responsibilities:
- Conduct thorough assessments to identify patient and family needs.
- Collaborate with the IDG to develop and implement individualized Plans of Care.
- Provide education to patients, families, and caregivers regarding disease processes and palliative care interventions.
- Initiate skilled nursing interventions to enhance comfort and prevent complications.
- Document all care provided in accordance with company policies.
- Act as a representative of the organization at assigned facilities, facilitating referrals and maintaining professional relationships.
- Engage in quality assessment and performance improvement initiatives.
Qualifications:
- Minimum of one (1) year of inpatient nursing or hospice experience for external candidates; internal applicants must have at least six (6) months of LPN experience in hospice or home health.
- Proficient in using EMR/EHR systems.
- Valid driver's license and automobile insurance as per company policy.
- Ability to communicate effectively in English, both verbally and in writing.
- Flexibility in scheduling to accommodate the needs of patients.
- Physical capability to perform essential job functions, including lifting and mobility tasks.
Additional Requirements:
- Current BLS certification for healthcare professionals.
- Compliance with all relevant regulations and company policies.
- Willingness to participate in ongoing training and professional development.
As a member of our team, you will provide and manage direct care to patients and families, ensuring a holistic approach that encompasses psychosocial, spiritual, and physical needs. Your expertise will contribute to the comfort and dignity of those we serve.
