Job Summary

We are seeking a highly skilled and dedicated Home Visitor Program Coordinator to join our team at Nationwide Children's Hospital. The successful candidate will be responsible for developing, coordinating, and implementing programs that target specific populations, while practicing professional social work and promoting advocacy in accordance with the NASW Code of Ethics for Social Work.

Key Responsibilities:

  • Manage pertinent expenses, budget items, data collection, evaluations, and personnel matters to accomplish program goals and objectives.
  • Ensure the provision of clinical services, including care coordination, to facilitate the goals and objectives of the Clinical Medical Social Work department.
  • Explore and secure alternative funding sources and serve as a liaison to various community organizations and corporations.
  • Collaborate with Clinical Medical Social Work leadership and with administrators of internal stakeholders and affiliated teams in the development, implementation, and evaluation of program components.

Requirements:

  • MSW degree
  • LISW license required, LISW-S preferred
