About the Role

We are seeking a highly motivated and experienced Community Integration Coordinator to join our PACT team in Boston/Arlington. As a key member of our team, you will be responsible for providing psychosocial support to individuals with psychiatric experiences and mental illness.

Key Responsibilities
  • Provide service coordination (case management) for an assigned group of clients
  • Develop, write, implement, evaluate, and revise overall treatment goals and plans in collaboration with the client and the ITT
  • Provide individual side-by-side support, counseling, therapy, and symptom management
Requirements
  • Master's degree in a relevant field
  • Post-secondary school education and training, including internships and other supervised practical experiences in a clinical or rehabilitation setting
  • Licensed or certified to work in the field, committed to a process of lifetime knowledge and skills development
