Key Responsibilities:

  • Provide personal care and hygiene assistance, including bathing, showering, incontinence care, Foley catheter management, and feeding residents according to their individualized care plans. Monitor and report output from drainage collection devices to the Nursing Team Leader.
  • Assist residents with mobility to and from the restroom using appropriate transfer techniques as outlined in their care plans.
  • Deliver oral and denture care as per facility protocols and directives.
  • Ensure residents are repositioned in bed and seated in chairs according to facility guidelines, including elevating heels as necessary.
  • Respond promptly to call lights and provide necessary assistance to residents, reporting any requests needing a licensed nurse's attention to the Nursing Team Leader.
  • Complete a Stop and Watch form and communicate any changes in the resident's condition to the Nursing Team Leader.
  • Demonstrate proficiency in obtaining and documenting vital signs, including temperature, pulse, respiration, weight, and intake/output, reporting findings to the Nursing Team Leader as assigned.
  • Assist with maintaining cleanliness in resident areas, including organizing personal hygiene items and ensuring a tidy environment.
  • Clean personal mobility equipment (wheelchairs, walkers) as required.
  • Document care provided to residents in the tracking system accurately.
  • Support residents in attending activities and may assist in facilitating the activity program as needed.
  • Prepare resident rooms for new admissions, orienting residents to their environment, call light, and providing necessary supplies while completing additional admission tasks as directed.
  • Assist residents with dressing and packing during discharge, ensuring the room is cleared of supplies, trash, and linens while completing additional discharge tasks as directed.
  • Organize work efficiently to complete assigned tasks in a timely manner.
  • Collaborate as part of a team with other staff members, participating in interdisciplinary care planning.
  • Uphold resident confidentiality and adhere to HIPAA regulations.
  • Exhibit professionalism in conduct and interactions with peers, staff, and residents.
  • Perform additional duties as assigned.
