Key Responsibilities:- Conduct in-home visits to assess patients' overall health and well-being
- Identify potential barriers to care and develop personalized plans to address them
- Collaborate with patients and their healthcare teams to ensure seamless care transitions
- Competitive compensation package, including a base rate of $80 per completed assessment
- Opportunities for additional compensation through our pay-per-test model
- Flexible scheduling to accommodate your needs
- Malpractice coverage and other benefits
- Master's degree in nursing or related field
- Current licensure as a nurse practitioner in Tennessee or Georgia (or willingness to obtain)
- Ability to travel throughout TN/GA and cover multiple counties
