Job Summary

We are seeking a Per Diem Medical Social Worker to join our team of dedicated professionals who are passionate about helping patients and families navigate the end-of-life journey. As a Medical Social Worker, you will be responsible for providing psychosocial evaluations, ongoing counseling, and emotional support to patients and families in need.

Responsibilities
  • Conduct thorough psychosocial evaluations to identify patient and family needs
  • Develop and implement individualized plans of care to address patient and family concerns
  • Provide ongoing counseling and emotional support to patients and families
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Document patient and family interactions and progress in a timely and accurate manner
Requirements
  • MS degree in social work from an accredited school of social work
  • 2+ years of clinical work experience, preferably in healthcare or hospice
  • Ability to sit, stand, bend, move intermittently, and lift at least 25lbs
