About This Opportunity

We are seeking an experienced Physical Therapist to join our Home Health team at Kaiser Permanente in Union City, California. This is a unique opportunity to work with patients in their place of residence and make a meaningful impact on their health and well-being.

Job Summary:

This role involves providing independent physical therapy services to patients in their homes, teaching them preventative and rehabilitative procedures, and developing and coordinating plans of care. You will be working closely with other healthcare professionals to ensure that patients receive the best possible care.

Key Responsibilities:
  • Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction.
  • Establish treatment goals and plans treatment to achieve established goals.
  • Initiate discharge planning for physical therapy and implement the plan of care through direct treatment of the patient.
  • Assess for adaptive equipment needs and coordinate assessments with certified vendors as needed.
Requirements:
  • Minimum two (2) years of experience in physical therapy.
  • Graduate of an accredited physical therapy curriculum with a bachelors, masters or doctoral degree.
  • Physical Therapist License (California) required at hire.
  • Driver's License (California).
  • National Provider Identifier required at hire.
  • Basic Life Support.
