We are seeking a skilled Staff Nurse to join our Home Care Services Program (HCSP) team. As a Staff Nurse, you will be responsible for conducting in-home assessments, determining the level of home care services required, and developing plans of care for clients.

Key Responsibilities
  • Conduct in-home assessments to evaluate clients' ability to perform activities of daily living and determine the degree of assistance required.
  • Determine the level of home care services or other services of care needed based on medical diagnosis and functional impairment.
  • Develop plans of care that take into consideration clients' home environment and social support systems.
  • Complete home visits and submit assessments and recommendations within five workdays.
  • Conduct case reviews, participate in special projects, and provide consultation to HRA Medical Review Team and social service staff.
  • Prepare documentation for and testify at Fair Hearings and other legal proceedings.
Requirements
  • A valid New York State Registered Nurse License.
Preferred Skills
  • Excellent communication and assessment skills.
  • Ability to utilize computer programs and enter data in an electronic field visit report.
  • Bilingual ability.
  • Time management skills.

As a Staff Nurse with the City of New York, you may be eligible for federal loan forgiveness programs and state repayment assistance programs.

Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
