Job Title: Floater LPN/RN

At TLC Nursing Center, we are seeking a compassionate and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our healthcare team as a floater. As a key member of our team, you will play a vital role in assessing, planning, implementing, and evaluating nursing care for residents based on their individual needs.

Key Responsibilities:
  • Assess residents' physical, emotional, and psychosocial needs and develop individualized care plans in collaboration with the interdisciplinary team.
  • Administer medications and treatments as prescribed, monitor residents for any adverse reactions, and provide education on medication management and side effects.
  • Monitor residents' vital signs, symptoms, and changes in condition, and initiate appropriate interventions as needed.
  • Provide direct nursing care, including wound care, catheter care, and assistance with activities of daily living.
  • Supervise and delegate tasks to certified nursing assistants (CNAs) to ensure the delivery of safe and effective care.
  • Document resident assessments, care plans, interventions, and outcomes accurately and timely in accordance with regulatory standards and facility policies.
  • Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives to enhance resident care and outcomes.
