Clinical Care Specialist

We are seeking a skilled and compassionate Clinic Nurse to join our team at Notch. As an integral part of our clinic team, you will work closely with our providers and support staff to provide clinical assistance and contribute to the effective delivery of care.
Key Responsibilities- Perform patient intake, including vital signs, and update medical data, medications, and health problems in charts.
- Triage telephone calls regarding medical concerns and assist medical providers during procedures and exams.
- Handle referrals and phone calls to other provider offices for appointments, and make callbacks or follow-ups for medical concerns as necessary.
- Collect specimens for outside laboratory testing and perform basic in-house laboratory tests.
- Initiate and/or execute age-appropriate preventive care measures, engage patients in self-management action planning, and assist providers with patient education and instructions.
- Effectively communicate with patients using techniques such as motivational interviewing when appropriate.
- Conduct home visits for duties within the scope of practice as determined by the Vermont State Board of Nursing when necessary.
- Verify presence of superbill, check charts for insurance changes and patient information, prepare prior authorizations, and transcribe telephone messages from prescription lines.
- Maintain appropriate levels of unexpired medical supplies, manage record requests, and send clinical information for referrals.
- Complete Quality Assurance tasks, maintain sanitation of exam rooms and equipment, and verify up-to-date vaccinations on the Vermont Department of Health website.
- Conduct Medicaid and Sliding Fee chart reviews, monitor fax machines, and distribute faxes.
- Participate in Quality Improvement projects, training sessions, and staff meetings, and train, mentor, and supervise students and clinical support staff as appropriate.
- LPN or RN licensure required.
- Associates or Bachelors Degree in Nursing preferred.
- 1 year clinical experience preferred.
- Ability to perform venipuncture, administer injections, and complete CLIA waived lab tests.
- Knowledge of correct specimen handling procedures, medical terminology, OSHA manual and standards, and billing codes and procedures.
- Ability to juggle multiple tasks, prioritize workload, and function within a team environment.
The Clinic Nurse will work in a fast-paced clinical environment, interacting with patients and members of the care team to provide a range of care services. The employee is occasionally exposed to x-ray machines and blood-borne pathogens and often exposed to patients with infectious diseases.
