- Manage Care Plans: Develop, implement, and evaluate care plans in accordance with regulatory requirements and best practices.
- Stay Current with Industry Developments: Remain up-to-date with the latest industry trends, regulations, and best practices in care management and resident assessment.
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a strong clinical background and experience in care management.
- AANC Certification (AANC-CT) or willingness to obtain within a specified timeframe.
- Knowledge of Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS) principles.
