Responsibilities- Develop and implement personalized care plans in accordance with patient needs
- Conduct home visits to clients in designated geographic territories, providing high-quality care and support
- Administer medication, perform wound care, treatments, and procedures as assigned
- Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
- Execute and properly document doctors' orders, ensuring accurate and timely care
- Perform client assessments as necessary, utilizing a state-of-the-art touch pad tablet
- Coordinate case management and services, ensuring seamless care delivery
- Graduation from an accredited and approved nursing program
- A current license as a Registered Nurse in New Jersey
