About the Role:

  • You will serve as a patient advocate, working closely with patients, families, and healthcare providers to coordinate care delivery in the home or community setting.">
  • Your expertise will ensure that patient care plans are comprehensive, timely, and appropriate, with ongoing evaluation and modification as needed.">
  • You will communicate effectively with the multidisciplinary team, support services, and community resources to achieve desired clinical outcomes in an efficient and cost-effective manner.">
  • You will maintain accurate and complete documentation, ensuring reimbursement and compliance with regulatory requirements.">
  • As a seasoned nurse educator, you will precept newly hired nurses, sharing your knowledge and expertise to enhance their skills and confidence.">

Requirements:
