**Job Summary:**

We are seeking a dedicated and compassionate Medical Professional - Patient Care Specialist to join our team at Intercare Health Network. In this role, you will be responsible for providing high-quality patient care to individuals of all ages, from pediatrics to internal medicine.

About the Role:

  • As a Medical Professional - Patient Care Specialist, you will work collaboratively with our healthcare team to deliver exceptional patient outcomes.
  • You will be responsible for conducting thorough medical assessments, developing personalized treatment plans, and coordinating care with other healthcare professionals.
  • Your expertise in electronic health records (EHRs) will enable you to accurately document patient information and ensure seamless communication among team members.

Requirements:

  • A current license to practice as a registered nurse or physician assistant in Michigan, as applicable, along with specialty certification as a nurse practitioner.
  • DEA and Controlled Substances Certification/license, as well as all other LIP credentialing and privileging requirements.
  • BLS certification and thorough working knowledge of prevailing standards of care, InterCare's clinical guidelines, patient care policies and procedures, and applicable accreditation and PCMH standards.

Work Environment:

  • Our offices are situated in a typical office environment, requiring the ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 pounds.
  • Excellent hand-eye coordination, reach with arms & hands, and finger dexterity are necessary for tasks involving keyboard use, office equipment operation, and printed material reading.

Salary and Benefits:

The estimated annual salary for this position is $85,000-$110,000, depending on qualifications and experience. Our comprehensive benefits package includes medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
