We are seeking an experienced Registered Nurse to join our team as a Home Health Registered Nurse - Private Care Professional. This is a unique opportunity to provide high-acuity, private care to patients in the comfort of their own homes.
Key Responsibilities- Participate in admissions and perform timely start of care visits for new patients to the program.
- Conduct comprehensive patient assessments and provide patient care, including medication reconciliation, Trach, IV, colostomy, and surgical incision assessments and treatments.
- Collaborate with the treating clinical team through regular calls and provide therapeutic interventions as needed.
- Administer prescribed medications, obtain blood samples, and educate patients on how to manage their illness, prescribed medications, and care plan.
- Complete private visits to frail, adult populations in a home setting with support from the clinical team.
- Associate of Science in Nursing, BSN preferred.
- Current, unencumbered state license to practice as a Registered Nurse.
- 1 year of adult, acute, inpatient medical, or medical/surgical care experience within the last five years.
- Current CPR certification.
- Ability to use public transit for visits.
- Experience providing RN services in the home, community, or clinic, preferred.
- Adult medical critical care, preferred.
Competitive salary range: $120,000-$130,000 and comprehensive benefits package.
