Job Title: Licensed Practical Nurse (LPN)Job Summary

TEEMA Solutions is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care and support in a professional and prescribed manner at a state long-term care facility in Tucson, Arizona.

Responsibilities
  • Administer medications and perform various treatments as ordered by physicians within your scope of license.
  • Observe and report the effectiveness of medications and treatments.
  • Ensure adequate supplies and medications are available for resident care.
  • Complete initial admission nursing assessments for new residents.
  • Assist in the development of Minimum Data Set (MDS) assessments and care plans.
  • Maintain accurate clinical and administrative documentation.
  • Supervise and assist subordinate staff with resident care, including Activities of Daily Living (ADLs).
  • Actively participate in the Arizona Management System (AMS) for problem-solving and work process improvements.
  • Contribute to our Quality Assurance and Performance Improvement (QAPI) program.
  • Conduct in-service training and staff development programs.
  • Attend and engage in staff meetings.
Qualifications
  • Valid LPN license issued by the state of Arizona.
  • Nursing Home experience required.
  • BLS issued by the AHA.
  • Current AZ Fingerprint Card.
  • All state requirements to practice as an LPN in AZ.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with interdisciplinary teams.
  • Proficiency in clinical documentation.
  • Commitment to continuous improvement and professional development.
