Job Summary:We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in their homes or alternate community settings.
Responsibilities:- Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
- Develop and implement personalized plans of action in collaboration with physicians to achieve optimal patient outcomes.
- Document all changes in patient condition and update care plans accordingly.
- Identify counseling needs and plan appropriate teaching strategies to promote patient education and empowerment.
- Monitor the quality and appropriateness of services provided to ensure the highest level of care.
- Adhere to agency infection control and safety policies to maintain a safe and healthy environment for patients and staff.
- Meet the organization's policy requirements for absenteeism and appearance.
- Physical Therapy Licensure in New Jersey is required.
- 2-3 years of prior clinical experience and community-based experience are preferred.
- Strong computer skills and effective communication and interpersonal skills are essential.
- A valid New Jersey driver's license is required.
This role requires working indoors and outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, and the use of a phone and tablet for documentation.
