Key Responsibilities:- Assess the needs of displaced clients and families, providing comprehensive housing plans to help them become self-sufficient.
- Meet with clients at their homes or current residences, developing and implementing housing plans that address their unique needs.
- Coordinate necessary services, including crisis intervention, short-term housing counseling, and referrals to community resources.
- Engage in outreach activities, building relationships with community partners and stakeholders to ensure seamless service delivery.
- Ensure compliance with Department of Housing and Urban Development policies, maintaining accurate client records and providing regular updates to program managers.
- Minimum two years of case management experience, with a strong understanding of homelessness, addiction, and mental health issues.
- Demonstrated communication skills and cultural sensitivity, with the ability to work effectively with diverse populations.
- Proficiency in Microsoft Office, including PowerPoint, Word, and Excel, with experience using computer-aided tools for data entry and record-keeping.
- Bilingual (Spanish) preferred, with knowledge of motivational interviewing techniques.
- A valid California driver's license, proof of insurance, and a reliable personal vehicle for off-site travel.
- A competitive hourly rate of $22.04, with comprehensive benefits, including healthcare coverage, employer-matched 401(k), and life insurance.
