**Job Description:**

We are seeking a skilled Patient Care Coordinator - Medication Support to join our team at the University of Texas Medical Branch. This role involves providing technical support in patient care, ensuring the well-being and safety of our patients.

The ideal candidate will have a strong understanding of healthcare protocols and be able to work effectively in a fast-paced environment. As a Patient Care Coordinator - Medication Support, you will assist professional staff in maintaining a safe therapeutic environment, collect and report vital signs, and participate in general health care activities.

Earn an estimated $55,000 - $75,000 per year, depending on experience, with opportunities for growth and professional development.

Responsibilities:

  • Coordinates patient care activities and provides technical support to ensure the well-being and safety of patients.
  • Assists with patient activities and care as assigned by the charge nurse/designee as indicated by the patient's plan of care.
  • Provides for patient's personal hygiene; assists with chronic wound care; assists with feeding; and ambulates, positions, and turns patients with altered mobility.
  • Maintains accurate records of patient data, including vital signs, height and weight, intake and output, pulse oximetry, and collected specimens.
  • Collaborates with healthcare professionals to ensure effective patient care and maintain a safe therapeutic environment.
  • Participates in in-service orientation and continuing education classes. Obtains 40 hours of continuing education/in-service hours on an annual basis to meet ACA standards.
  • Adheres to internal controls and reporting structures.
  • Performs related duties as required.

Requirements:

  • High school or GED equivalent.
  • Current Texas Certified Medication Aide permit.
