Job Overview

We are seeking an experienced Nurse Practitioner (Temp) to join our team at Greater Good Health in Santa Clara, CA. As a Clinical Care Manager, you will lead patient care through telehealth and occasional in-home visits, providing exceptional patient experiences and working closely with the care team to deliver holistic, high-touch, coordinated care for a panel of senior patients.

About the Role

This role requires strong clinical skills, excellent communication, and a passion for delivering high-quality patient care. You will work closely with the care team to manage patient panels, provide education, and coordinate care to ensure optimal outcomes.

Key Responsibilities
  • Patient Assessment and Care
    • Conduct thorough assessments of patients' medical history, physical condition, and social determinants of health
    • Develop and implement individualized care plans that address patients' unique needs and goals
  • Clinical Decision-Making
    • Makes informed decisions about diagnosis, treatment, and referrals based on current research and best practices
    • Evaluates and interprets laboratory results, imaging studies, and other diagnostic information
  • Collaboration and Communication
    • Work collaboratively with interdisciplinary teams to coordinate patient care and ensure seamless transitions
    • Communicate effectively with patients, families, and caregivers to educate them about their care and promote self-management
Requirements
  • Current licensure as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) in the state of California
  • CPR/BLS certification and prescriptive authority by the CA state board of Nursing or Medical Examiners and DEA license
Salary Range

$145,000 - $165,000 per year, depending on experience.
