Per Diem Nurse Practitioner/Physician Assistant

Role OverviewWe're seeking a highly skilled Per Diem Nurse Practitioner/Physician Assistant to join our Senior Community Care team. As a key member of our team, you'll provide exceptional care to Optum members, delivering medical services in a periodic or intermittent basis.
Key Responsibilities- Deliver high-quality, cost-effective care to assigned members
- Manage medical and behavioral conditions effectively, collaborating with physicians or specialty providers
- Perform comprehensive assessments and document findings in a concise, compliant manner
- Ensure accurate ICD10 coding and documentation
- Facilitate agreement and implementation of the member's plan of care
- Coordinate care as members transition through different levels of care and care settings
- Monitor the needs of members and families, making adjustments to the plan of care as situations change
- Certified Nurse Practitioner through a national board
- Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
- Active and unrestricted license in the state of practice
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
- Ability to work under a Collaborative Agreement in applicable states
- 1+ years of hands-on post-grad experience within Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
- Proficient computer skills, including documentation of medical information with written and electronic medical records
