Job Title: Community Health Worker

Job Summary

The Community Health Worker will work closely with our clinical providers, medical assistants, and scheduling team to ensure seamless patient care. This role requires strong communication and interpersonal skills, as well as the ability to work in a fast-paced environment and prioritize multiple tasks.

Key Responsibilities
  • Gather and analyze patient health data to identify areas for improvement and develop targeted interventions.
  • Schedule in-person or telemedicine visits with patients to improve access to healthcare and social services.
  • Design and implement plans with care teams to improve patient health outcomes.
  • Identify high-risk individuals and assess for chronic diseases, providing education and support to promote healthy behaviors.
  • Manage complex care needs of patients, providing access to resources and services to ensure optimal care.
  • Assess patients' social determinants of health, using screening tools and questionnaires to identify areas for improvement.
  • Act as a patient advocate and health navigator, assisting patients in understanding their healthcare options and accessing necessary resources.
  • Document all patient interactions and care plans in our electronic health record system.
Requirements
  • High school diploma or equivalent required; associate's degree preferred.
  • Six months of medical documentation experience or training required.
  • Proficiency in Microsoft Office Suite required.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Strong communication and interpersonal skills.
  • Ability to think critically and make sound decisions.
  • Ability to use technology to optimize efficiency and effectiveness.
What We Offer

We offer a competitive salary of $21.50 per hour, based on experience and THW certification. Our team is dedicated to providing exceptional care and support to our patients, and we are committed to creating a positive and inclusive work environment.
