About the Role

Elderwood at Hamburg is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in ensuring the health and well-being of our residents by providing comprehensive and quality therapy services.

Responsibilities
  • Initiate skilled physical therapy treatments in accordance with physician orders and maintain required contact with the physician.
  • Maintain written evaluations, progress notes, and discharge summaries for individuals in the physical therapy program.
  • Confer with other healthcare staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
  • Provide input for meetings and attend during absences of the Director of Physical Therapy.
  • Maintain statistical records of individuals' treatments.
  • Assist with in-service training programs upon request.
  • Record and update individuals' plan of care in the Inter-disciplinary Kardex.
  • Submit requisitions for supplies needed in the treatment to the Director of Physical Therapy.
Requirements
  • Graduation from an approved school of physical therapy is required.
  • A state license to practice physical therapy is required.
  • Two years of experience in physical therapy is preferred.
  • The ability to direct others is necessary.
