Job Summary

CVS Health is seeking a skilled Case Management Coordinator to join our team. As a key member of our healthcare team, you will play a critical role in delivering exceptional patient care and outcomes.

Key Responsibilities
  • Evaluate member needs and develop comprehensive care plans to ensure optimal health outcomes.
  • Collaborate with healthcare providers, community organizations, and members to address social determinants of health and promote healthy behaviors.
  • Utilize case management tools and resources to identify high-risk factors and develop strategies to mitigate them.
  • Coordinate and implement care plans, monitoring progress and making adjustments as needed.
  • Enhance medical appropriateness and quality of care by consulting with case managers, supervisors, and medical directors.
Requirements
  • 2+ years of experience in behavioral health or social services.
  • Ability to work 8:00a-5:00p in the assigned market time zone.
  • Proficiency in Microsoft Office applications (Word, Excel, Outlook).
Preferred Qualifications
  • Case management and discharge planning experience.
  • Managed Care experience.
Education

Bachelor's degree or non-licensed master level clinician with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling) or equivalent experience.
