We are seeking a highly skilled and compassionate Case Manager to join our team at the Mental Health Association - Western MA. As a Case Manager, you will play a critical role in providing support and services to individuals with mental health and substance use challenges.
Key Responsibilities- Provide outreach, assessment, and engagement to participants of the BSAS Low-Threshold Housing Program using a trauma-informed, person-centered approach.
- Support individuals in transitioning into housing, maintaining housing, and ensuring their other needs are met.
- Provide emotional support and psychoeducation on coping skills and harm-reduction strategies.
- Complete documentation thoroughly and in a timely manner.
- Perform responsibilities professionally, in accordance with MHA's Core Values (Respect, Integrity, and Compassion), policies, practices, program funding, and applicable regulatory agency guidelines.
- Intake Coordination: Coordinate documentation necessary for entry into the program, including homeless verification, substance use assessment, eligibility screening, authorizations, and notice of policies.
- Service Planning: Serve as a member of the multidisciplinary team in the development of the Individualized Action Plan, utilizing the participant's strengths in establishing and meeting their goals.
- Housing Stability: Assist participants in entering safe housing, develop and maintain relationships with landlords, and provide tenancy support.
- Income and Benefit Maximization, Employment: Assist participants in securing and maintaining employment, employment training, or education, and educate and assist participants to understand and access benefits and resources.
- Community Resources and Access: Promote participant engagement in community life and resources, assist with access to libraries, recovery groups, social activities, day/vocational programming, and local events.
- Participant Health and Wellbeing: Provide emotional support, support the development and practicing of coping skills, and counsel on harm-reduction strategies.
- Valid driver's license, sufficient automobile insurance, acceptable driving record, and a safe reliable vehicle during working hours.
- Demonstrated ability to work independently, manage time, prioritize tasks, take initiative, follow through, and work cooperatively as a member of a team.
- Good oral, written, computer, and organizational skills.
- Demonstrated ability to adapt to the changing needs of the program participants.
- Strong interpersonal skills, patience, caring, and compassion.
- Must be able to pass and maintain certifications in CPR and first aid.
- Must be at least twenty-one years old.
- Bachelor's degree in human services or relevant field required.
- Requires six months of applicable experience.
