Job Title: Licensed Practical Nurse
Location: Pooler, Georgia
Estimated Salary: $50,000 - $70,000 per year
Job DescriptionIn this role as a Licensed Practical Nurse (LPN) at SouthCoast Health, you will have the opportunity to work in a dynamic and fast-paced environment providing high-quality patient care. As an integral member of our healthcare team, you will be responsible for assessing patients' general condition, particularly the acutely ill patient, and ensuring that their needs are met.
You will assist physicians with examinations, diagnostic procedures, and treatments, as well as provide direct patient care as ordered by the provider. Maintaining confidentiality is essential in matters regarding the clinic, its patients, and employees.
The successful candidate will have excellent communication skills, be able to work effectively in a team environment, and demonstrate a commitment to providing exceptional patient care.
Responsibilities- Accurately document all patient observations, treatments, and procedures administered, patient/family responses, medication dispensed or prescribed, and hospital data in the medical record.
- Promote wellness by providing patient education materials, communicating physicians' advice, and instructions on how to self-manage care.
- Coordinate patient care by triaging patient phone calls regarding urgency of appointments and scheduling hospital admissions.
- Process all patient referrals and required follow-through, maintaining and keeping updated referral reports.
- Schedule lab work, specialist visits, complete necessary paperwork to ensure proper billing, complete physician orders, verify, and update patient data.
- Assist patients with completion of paperwork, as required.
- Assist with patient inquiries/concerns regarding medical care, medication instructions, and prescription call-ins in a prompt, courteous, and efficient manner.
- Assist with patient plans of care and document care coordination and interventions while integrating quality initiative measurements and other accrediting body requirements.
- Coordinate care with hospital, home health services, hospice, or other institutional settings per provider's order.
- Administer immunizations and injections after reviewing immunization records, potential allergic reactions, identifying and reviewing side effects, and treatments.
- Current Georgia LPN or Compact licensure.
- Pass proficiency test.
- A current BLS/CPR certification.
