The Intake Coordinator plays a crucial role within the admissions team at Brook Lane Inc, responsible for processing individuals into our healthcare system. This position is essential for conducting initial assessments and emergency evaluations to effectively triage patients, ensuring they receive the appropriate care and referrals to community resources.

PRIMARY DUTIES:

  • Conducts comprehensive intake assessments with patients, guardians, and relevant contacts to identify their challenges and symptoms prompting the need for assistance.
  • Skillfully evaluates patient symptoms, risk factors, and requirements to align them with the most suitable services, whether at Brook Lane or through external community partners.
  • Maintains accurate records of services rendered in accordance with organizational policies.
  • Communicates pertinent information to the appropriate departments and provides timely feedback to referring parties.
  • Possesses a broad understanding of Brook Lane Health Services' offerings to address inquiries from clients and the public effectively.
  • Stays informed about community resources to facilitate connections for patients requiring additional support not available at Brook Lane.
  • Demonstrates exceptional customer service by fostering positive relationships with all stakeholders, including patients, colleagues, and external agencies.

REQUIREMENTS:

  • A bachelor's degree in social work or counseling from an accredited institution.
  • Preferred: Licensed Bachelor Social Worker (LBSW) in Maryland, though not mandatory.
  • Experience or training in working with specialized populations is advantageous.
  • Ability to engage effectively with diverse individuals and situations.
  • Capability to provide therapeutic support to individuals in crisis.
