GENERAL OVERVIEW:


The Clinical Pharmacist is accountable for delivering comprehensive, patient-centered pharmaceutical services during their designated shifts across various areas of the pharmacy department.

This role operates under the guidance of the administrative or clinical coordinator and oversees the activities of support staff during their shift.

The pharmacist is expected to demonstrate effective management and clinical judgment, making informed decisions when coordinators or senior staff are unavailable.

They collaborate with coordinators to identify and address issues within their practice area and contribute to problem-solving efforts.

The pharmacist also plays a key role in mentoring pharmacy interns and externs across all pharmacy service sectors.

KEY RESPONSIBILITIES:
Facilitates the preparation and distribution of unit dose medications, intravenous solutions, investigational drugs, and other pharmaceuticals to support patient care needs.

Assesses and monitors the medication requirements of patients.

Examines patient records and engages with patients, physicians, or nursing staff to gather information for medication profiles. Analyzes drug-related data to identify potential therapy-related issues and proposes suitable resolutions.

Confirms medication orders for accuracy and compliance. Reviews and verifies prepared medications for correct dosage, administration route, frequency, and treatment duration.

Provides direct oversight of support personnel assigned to their shift.

Participates in the education and training of pharmacy students, residents, fellows, and support staff, both in practical and theoretical settings.

Offers drug and poison information to patients, healthcare providers, and nursing staff.

Conducts timely reviews of nursing units as assigned.

Assists in evaluating the performance of support personnel in the area.

Continuously assesses and monitors patients' responses to prescribed medications in accordance with established practice standards. Provides pharmacokinetic dosing consultations as necessary.

EDUCATION AND EXPERIENCE:


A degree from an accredited college of pharmacy is required, along with completion of an internship and/or residency in hospital pharmacy.


Strong verbal and written communication skills are essential for effectively disseminating drug information to all levels of healthcare personnel.


A comprehensive understanding of disease pathophysiology and pharmacotherapy is necessary, along with the ability to integrate this knowledge into monitoring drug therapy.


Skills for participating in teaching programs, including instruction and supervision of pharmacy students, interns, and pharmacy technician trainees, are required.

Capability to provide both informal and formal instruction on drug therapy and operational activities within the inpatient setting is essential.

The ability to collaborate with others to enhance patient services is a must.

CERTIFICATIONS/LICENSES REQUIRED:
Must hold a valid license or be eligible for licensure in the State of Michigan.

Licensure must be obtained within 30 days of hire; failure to secure licensure will result in termination of the pharmacist position.
