Key Responsibilities:
- Coordinate services throughout the continuum of care, ensuring quality patient care while utilizing resources effectively.
- Conduct concurrent clinical reviews to assess the appropriateness of admissions and the ongoing need for care in an acute hospital environment.
- Facilitate collaboration and communication among patients, families, and the healthcare team regarding care plans and post-discharge needs.
- Perform telephonic clinical reviews with insurance claims personnel to support reimbursement for patient care services.
- Identify quality and risk issues, reporting them to the relevant departments.
Qualifications:
- Active Registered Nurse licensure in the State of Colorado or a current multistate Registered Nurse license.
- Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
- Case Management or CPHQ Certification is highly preferred.
- Graduate of an accredited nursing program; a Bachelor of Science in Nursing (BSN) is desirable.
- Minimum of 2 years of clinical nursing experience in an acute care setting, demonstrating skills in assessing, planning, and managing patient care delivery.
- Previous experience in utilization review, quality assurance, discharge planning, or case management is strongly preferred.
