About the Role

In this Speech Therapist position, you will work closely with patients in the Gainesville, FL area, providing personalized support and guidance to help them achieve their full potential.

Your primary focus will be on restoring motion and ensuring each client's functional abilities are maximized, allowing them to live safely and comfortably in their own homes.

You will work collaboratively with healthcare professionals to develop and implement effective treatment plans, promoting the quality of life and movement potential of our patients.

Under the direction of a physician and in consultation with patients and caregivers, you will play a vital role in ensuring a better quality of life and healthy recovery for our patients.

Requirements
  • Current licensure in the State of FL and completion of fingerprinting
  • At least two years of experience in Home Health, preferably
  • Alignment with the Agency's philosophy and goals
  • Ability to exercise initiative and independent judgment
Responsibilities
  • Understand and adhere to established policies and procedures
  • Improve or minimize residual physical disabilities of patients
  • Return patients to an optimum and productive level within their capabilities
  • Participate in patient care planning with other healthcare professionals
  • Take initial histories and make initial evaluations
  • Perform skilled procedures as ordered by physicians
  • Consult with physicians regarding changes in treatment
  • Attend patient care conferences as scheduled
  • Prepare clinical and progress notes
  • Help develop and revise the plan of care as necessary
  • Complete and submit OASIS assessments, reassessments, and transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined time frames
