Job Overview

Lowe's Guardian Angel is seeking a highly skilled and compassionate Direct Support Professional III to join our team. This role involves providing exceptional support to individuals with intellectual and developmental disabilities in a Community Living Arrangement (CLA) setting.

Key Responsibilities

  • Behavioral Support & Supervision:
    • Counsel and manage behaviors using positive reinforcement and programming.
    • Assist with the prevention and de-escalation of aggressive behaviors, following ISP guidelines and instructions from behavioral specialists.
  • Health Monitoring & Documentation:
    • Monitor and document individual reactions to daily activities.
    • Record health and clinical data, such as vital signs, bowel movements, hydration, sleep logs, and other relevant health information.
    • Perform body scans and skin checks during each shift, reporting any deviations from health baselines to the Residential Manager.
    • Check Medication Administration Records (MARs) for accuracy and report discrepancies.
  • Individual Care & Daily Living Assistance:
    • Assist individuals with Activities of Daily Living (ADLs), including oral care, nail care, bathing, dressing, and toileting.
    • Help with meal preparation, feeding, and monitoring during mealtime.
    • Provide assistance with ambulation and transfers (e.g., moving individuals on/off commode and turning/positioning to prevent bedsores).
  • Training & Community Support:
    • Train individuals on prescribed skills at home and in the community as outlined in their ISP.
    • Provide input and feedback on the development and evaluation of the individual's ISP.
    • Monitor and assist with self-administration of medication (SSAM).
  • General Housekeeping & Communication:
    • Clean and organize living environments, including cleaning soiled linens and personal areas.
    • Relay important information to the next staff member on duty and complete shift handovers.

Qualifications

  • Education & Experience:
    • Must be 18 years or older.
    • Minimum of a high school diploma or GED.
    • Must pass a criminal background check and provide satisfactory TB screening upon employment.
  • Licensing & Certification:
    • CPI (Crisis Prevention Intervention) and CPR/First Aid/AED certifications required (must be obtained within the first 30 days if not already certified).
    • Must pass a hands-on competency exam for the DSP III role.
  • Physical Requirements:
    • Ability to perform physically demanding tasks, including standing, bending, kneeling, and lifting up to 50 pounds independently.
    • Ability to walk, sit, and ride in vehicles for extended periods.

Working Conditions

Working in a personal home environment and/or Day Program.

Responsible for assisting with feeding, oral care, hygiene, toileting, and overall daily care.

Cleaning and maintaining the living environment.

Recording clinical data and completing progress notes by the end of each shift.

Attending required training sessions and demonstrating competence in job duties.

Ensuring the health and safety of individuals by reporting changes to the Nurse and Residential Manager.

Additional Duties

Attend all required team meetings and ongoing education and training sessions.

Perform any other duties as assigned.
