LPN Position in Long Term Care Facility

Job Summary:
Amergis is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Long Term Care/Assisted Living Facility. As an LPN, you will be responsible for providing high-quality patient care, utilizing the nursing process to assess, plan, implement, and evaluate patient care.
Responsibilities:
- Perform focused interviews to identify specific patient needs
- Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses
- Develop and implement individualized patient plans of care, prioritizing actions based on patient care requirements
- Monitor consistency between patient plans of care and actions of medical staff
- Perform interventions according to identified priorities, plans of care, and facility policies and patient outcome standards
- Provide a safe, comfortable, and therapeutic patient environment
- Initiate and evaluate patient and/or family education
- Revised patient plans of care according to evaluation, changes in medical plans of care, and effective/ineffective nursing interventions
Requirements:
- Current Licensed Practical Nurse License for the state in which the nurse practices
- Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- One year prior professional nursing experience preferred
- Current BLS card
- Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater
- Successful completion of Background Screening Process
