We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Northwest Healthcare Home Health - Tucson. As a LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Provide high-quality clinical services to patients in their homes, following established protocols and procedures.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient visits thoroughly and accurately, maintaining up-to-date records and medication lists.
  • Collaborate with supervising RNs and other healthcare professionals to ensure comprehensive patient care.

Requirements:

  • Current LPN licensure in the state of Arizona.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle.
  • CPR certification.
