Key Responsibilities:- Provide comprehensive primary care to patients, including routine and acute visits, health risk assessments, and population health leadership.
- Collaborate with our care team to ensure seamless care coordination and effective communication with patients and specialists.
- Develop and implement personalized care plans that address the unique needs and health goals of each patient.
- Conduct home visits as needed to provide additional support and care to patients in their own homes.
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license and DEA license.
- US Work Authorization.
- Experience in primary care, internal medicine, geriatrics, or family medicine.
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
- Passion for teamwork and collaboration.
- Bilingual proficiency in applicable areas.

- 6 weeks of PTO, inclusive of vacation, sick time, major holidays, and continuing medical education (CME).
