Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health. As a key member of our care team, you will provide exceptional primary care to our patients, focusing on their unique needs and health goals.

Key Responsibilities:
  • Provide comprehensive primary care to patients, including routine and acute visits, health risk assessments, and population health leadership.
  • Collaborate with our care team to ensure seamless care coordination and effective communication with patients and specialists.
  • Develop and implement personalized care plans that address the unique needs and health goals of each patient.
  • Conduct home visits as needed to provide additional support and care to patients in their own homes.
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license and DEA license.
  • US Work Authorization.
Preferred Qualifications:
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration.
  • Bilingual proficiency in applicable areas.
